Sports Physical Information for 2023-24 School Year

In preparation for the 2023-24 school year, WISD is sharing information about required athletic paperwork for all WISD student-athletes. As in previous years, required athletic paperwork is given to student-athletes in the semester prior to the beginning of the athletic calendar year. This is done to make the beginning of the school year easier, and to ensure that students don't miss practice or competition events. All forms (except the UIL Preparticipation-Physical examination (PPE) and Medical History Form) must be submitted online to the athletic department prior to participation in any UIL Athletic event, including practice.
New this year, WISD will be using an online portal for the following forms:
- UIL Acknowledgement of Rules Form
- UIL Illegal Steroid Parent and Student Notification/Agreement Form
- UIL Concussion Acknowledgement Form
- Sudden Cardiac Arrest Awareness Form
The online portal system is managed by Register My Athlete/Aktivate. An instruction sheet regarding account creation and login details is located on the WISD website here.
The only forms that will be given out in paper form and returned to the athletic department will be the UIL Preparticipation-Physical Examination (PPE) and Medical History Form. These forms are linked online or can be downloaded once a student's account is created with Aktivate.
Coaches at both Danforth Junior High and Wimberley High School also have copies of the paper forms, as does WISD Athletic Trainer Johnny Gonzales. He can be emailed at johnny.gonzales@wimberleyisd.net and will send the forms. He can also be contacted at 512-847-5729, extension 3203.
Once completed, the paper forms can be turned into Coach Gonzales via email, turned in to the Fieldhouse, or dropped off at your child's school (DJH or WHS).
Physical Event on May 17
This year, WISD is hosting a sports physical event at Texan Gym on Wednesday, May 17 from 3:30 p.m. - 4:30 p.m. The event will be for incoming 7th, 9th, and 11th-graders (current 6th, 8th, and 10th-graders). The cost is $30.
To expedite the evening, WISD Athletics asks that all paperwork be filled out ahead of time. This will speed up the process and prevent delays. All medical release, medical history, medical emergency, and acknowledgement of rules forms must be completed before a sports physical can be performed. Athletes should also be appropriately dressed in a t-shirt and shorts. If you have any questions, please contact Johnny Gonzales, Athletic Trainer, at 847-3211, Ext. 3203.
