Sports Physicals

The University Interscholastic League (UIL) requires that all students who participate in athletic activities must undergo a physical exam and have all physical paperwork completed and submitted prior to participation in any practice, scrimmage, or game. The forms required are the:

  • UIL Preparticipation-Physical Evaluation Form (Medical History) *Filled out my parent/guardian*
  • UIL Preparticipation-Physical Evaluation Form (Physical Examination) *Filled out my medical professional*
  • UIL Acknowledgement of Rules Form *Available online*
  • UIL Illegal Steroid Parent and Student Notification/Agreement Form *Available online*
  • UIL Concussion Acknowledgement Form *Available online*
  • Sudden Cardiac Arrest Awareness Form *Available online*

The online forms are managed within the Rank One site. A detailed instruction sheet on setting up your student's Rank One account and moving over physical documents from Aktivate to Rank One is available in the Physical Packet linked below.

All forms (except the UIL Preparticipation-Physical examination (PPE) and Medical History Form) must be submitted online to the athletic department prior to participation in any UIL Athletic event, including practice.
